This page provides links to information and resources for professionals either interested in or currently contracting with the OPG for treatment services.
 

Contracting with the OPG

This link will take you to all the forms and information you need to pursue and maintain compliance with a contract for problem gambling treatment services.  Included in this section is: a FAQ for interested providers, terms and conditions, service rates paid to contractors, scope of work, application forms and renewal forms.

Provider Manual

This link will allow you to download or print the lates version of the OPG Provider Manual which defines provider and client eligibility, clinical records requirements, and billing procedures.

Forms

This page has links to all of the forms required by the OPG for reporting according to the contract for problem gambling services.  Forms can be downloaded or printed.  Included in this page are: billing forms, client intake form, client satisfaction survey, G-SAS, monthly evaluation form, and outreach request form.

Clinical Supervision

Here you will find information about clinical supervision.  For professionals interested in becoming an approved supervisor, you will be able to obtain a FAQ information sheet, and application form.  For professionals seeking supervision, a listing of approved supervisors with contact information is available.

Screening Tools

These tools can be used by any social service agency to screen their clients/customers/consumers for possible problem gambling behavior.  Although not exhaustive, these tools can be used by OPG contracted providers.  Tools that can be obtained here include:  Gamblers Anonymous 20 Questions, Gam-Anon 20 Questions, South Oaks Gambling Screen, National Opinion Research Center Diagnostic Screen (NODS) for gambling problems, and the NODS-CLiP (revision of the NODS).
